How to Register

Trying to Create an Account to Take Your PHA?

Welcome to URWell! If you have not already created an account, you will have to do so before you can complete your Personal Health Assessment (PHA).  Creating an account is fast, safe and easy. Just follow these steps!

  1. Click the yellow "Create an Account" button in the upper right-hand corner of the page.
  2. Check the box to accept the terms and conditions, then enter your name and date of birth, and click "find me".
  3. The system will look up your information. Confirm that it is correct. 
    1. If you are an employee, enter your NetID and NetID password
    2. If you are the spouse or domestic partner on an employee's insurance, enter your email address and create a password (this applies even if you are also employed by the University).
  4. The system will ask you to confirm a few details. If they are correct, click "yes".
  5. Click "Submit Registration".
  6. The system will send you a confirmation email, and you will be able to log in and take our online survey!

If you have difficulty with this process, try these helpful resources:

  • Be sure you have a current version (IE, Firefox, Safari, Chrome) that supports the website and PHA. If you need access to a computer, please call us at 275-6810.
  • Employees can test their NetID at https://myidentity.rochester.edu/.
  • If you are the dependent of another employee, you can reset your password using the "forgot password" tool on the Log In menu.  Just be sure to select "Spouse/Domestic Partner from the dropdown list.
  • Call 275-6810 Monday - Friday between 8:30 AM and 5:00 PM if you continue to experience difficulty.  A member of our staff will be happy to assist you!
