Program Eligibility

Those eligible for biometric screenings, the Personal Health Assessment (PHA), lifestyle management programs, condition management programs, and incentives include:

  • Regular full-time and part-time faculty, staff, residents, fellows, and spouses or domestic partners enrolled in a University health care plan
  • Non-Medicare-eligible retirees and spouses or domestic partners enrolled in a University health care plan
  • Postdocs and spouses or domestic partners enrolled in a University-sponsored health care plan

Those eligible for Behavioral Health Partners include:

  • Regular full-time and part-time faculty and staff and spouses or domestic partners enrolled in a University health care plan and their covered dependents 18 or older*
  • Non-Medicare-eligible retirees and spouses or domestic partners enrolled in a University health care plan and their covered dependents 18 or older*

*YOUR PPO Plan: Services received through BHP are not subject to the annual deductible and are covered at 100% by the Plan (i.e., there is no out-of-pocket cost). YOUR HSA-Eligible Plan: Services received through BHP are subject to the annual deductible and are covered at 100% after the deductible is met.

Please note, you are only eligible for programs and incentives if you are enrolled in a University or University-sponsored health care plan.

Eligibility for Life-Work Connections/EAP includes all employees and their immediate family members.
Eligibility for Well-U programs includes regular full-time, part-time faculty, staff, residents, fellows and postdocs.

Program Incentives

Getting healthy has its own rewards like feeling better, having more energy or experiencing less pain. As an added benefit, eligible individuals can receive up to $325 per year for completing certain activities and programs:

  • $125 per calendar year for completing the Personal Health Assessment, including a biometric screening, regardless of completion date.
  • $100 per calendar year for completing a lifestyle management program.
  • $100 per calendar year for completing a condition management program, if eligible.
