Atrial Fibrillation Management

Atrial Fibrillation (A Fib) is the most common heart rhythm problem among adults. The chance of developing A Fib increases with age and is more common if you have heart disease. If not treated, it can cause a stroke and can lead to cardiomyopathy, a weakening of the heart muscles that can eventually cause heart failure. It is important for you to work with your doctor to treat your A Fib. Our condition management nurses can help you manage this problem by:

  • Increasing knowledge of the disease
  • Providing strategies to slow disease progression and allow for self-management and lifestyle changes
  • Help managing co-morbid conditions which contribute to the chronic disease of A Fib (such as depression, anxiety or stress)
  • Review current medications and treatments
  • Help to develop personal goals and successful strategies to manage A fib

All of our condition management programs are personalized to meet your needs and schedule. You’ll work with a wellness coach who will meet with you one-on-one. Our nurses can work with you by phone, or with our easy-to-use telehealth software – secure web-based video conferencing you can access on your smartphone, tablet or any computer with a web-cam.
