Congestive Heart Failure Management


Congestive Heart Failure, or CHF, is a condition where the heart pumping function weakens and cannot pump enough blood to meet the body's needs. This is often a chronic, long-term condition and people can live with CHF for a very long time. A CHF diagnosis does not mean your heart has stopped working, but that the heart is not pumping as strongly as it should. If left untreated, CHF can lead to fluid retention in the legs, and even the lungs, and can cause breathing problems.

Our condition management nurses are ready to help individuals diagnosed with CHF manage their condition by:

  • Increasing knowledge of the disease
  • Reviewing the symptoms of CHF
  • Providing strategies for slowing disease progression, improving self-management and promoting lifestyle changes
  • Reviewing dietary information for individuals suffering from CHF
  • Managing co-morbid conditions contributing to the chronic disease of CHF (such as depression and anxiety)
  • Review current medications and treatments
  • Help to develop personal goals and successful self-management strategies to manage CHF

All of our condition management programs are personalized to meet your needs and schedule. You’ll work with a wellness coach who will meet with you one-on-one. Our nurses can work with you by phone, or with our easy-to-use telehealth software – secure web-based video conferencing you can access on your smartphone, tablet or any computer with a web-cam.
