Individual Programs

Keeping a healthy blood pressure can lower your risk of heart disease, kidney disease and stroke. Work with a dietitian over three sessions to lower your blood pressure through nutrition and activity changes.

A tobacco dependence counselor will help you over six sessions build a plan to manage withdrawal symptoms, prevent and manage relapses, and improve overall health.

Meet with a registered dietitian over three sessions to learn how nutrition and lifestyle impact your long-term health, and discover new ways to overcome your dietary challenges.

Available in-person and by telehealth.

After completing Fitness 101, work with a certified personal trainer over six sessions to develop a strength program designed to support your fitness goals. Sessions include assessments and guided workouts.

Available in-person only.

24 weeks – 18 group & 6 individual sessions

Receive tools, support, and encouragement to make and maintain lifestyle changes. Individuals must be eligible to participate; eligibility will be determined at enrollment.

12 weeks

Get help staying on track after completing the Weight Loss program. Receive continued nutrition, physical activity and behavior support for weight maintenance.

5 weeks

Build a foundational knowledge for physical activity by gaining an understanding of the various components of fitness, exploring motivational challenges, and learning how to prevent injuries.

5 weeks

Want to improve your health but not sure where to start? Review nutritional guidelines with a registered dietitian and explore new ways to incorporate heart-healthy foods into your diet.

4 weeks

Stress impacts more than just the day that you're having; it impacts your long-term health. Gain a better understanding of your stressors and make a plan to reduce them.

6-8 weeks

Explore the practice of living mindfully and using meditation to achieve balance, stability and personal satisfaction.

May include a day-long silent retreat.

Personal Wellness Coaching

Imagine what you can achieve with a personal wellness coach by your side! Personal wellness coaching for condition management is tailored to fit your individual needs and goals. You'll have the opportunity to work one-on-one with our team of wellness coaches including registered nurses certified in wellness, nutritionists, fitness specialists and stress and mindfulness experts.

In-person, telephonic and telehealth coaching options are available for most programs.

Learn how to avoid harmful triggers and properly use medications to breathe easier and feel better.

Participants will receive a prescription drug discount on eligible medications.

Learn how to avoid triggers for uncontrolled A-fib, and take necessary safety precautions to prevent complications.

Learn how to avoid flare-ups and to recognize warning signs that tell you to call your doctor. Learn how to modify daily activities to avoid breathlessness and minimize more lung damage.

Know your target blood pressure and cholesterol levels, and develop a plan to reach them. Create a diet and exercise plan to achieve and maintain a healthy weight. Learn how to manage your medications and keep your symptoms in check.

Identify lifestyle choices that will help manage symptoms of depression. Learn how to recognize triggers and to avoid choices that make symptoms worse. Life-Work Connections/EAP

Professional and confidential guidance provided at no cost for employees and their immediate family members. Life-Work Connections / EAP will help assess issues, and provide short-term counseling and referrals as needed with life events including:
