Those eligible for biometric screenings, the Personal Health Assessment (PHA), lifestyle management programs, condition management programs, and incentives include:
- Regular full-time and part-time faculty, staff, residents, fellows, and spouses or domestic partners enrolled in a University Health Care Plan
- Non-Medicare-eligible retirees and spouses or domestic partners enrolled in a University Health Care Plan
- Postdocs and spouses or domestic partners enrolled in a University-sponsored Health Care Plan
Those eligible for Behavioral Health Partners include:
- Regular full-time and part-time faculty and staff and spouses or domestic partners enrolled in a University Health Care Plan and their covered dependents 18 or older*
- Non-Medicare-eligible retirees and spouses or domestic partners enrolled in a University Health Care Plan and their covered dependents 18 or older*
*YOUR PPO Plan: Services received through BHP are not subject to the annual deductible and are covered at 100% by the Plan (i.e., there is no out-of-pocket cost). YOUR HSA-Eligible Plan: Services received through BHP are subject to the annual deductible and are covered at 100% after the deductible is met.

UR Medicine EAP includes all employees and household members including SEIU members not enrolled in a University Health Care Plan.
Eligibility for Well-U programs includes regular full-time, part-time faculty, staff, residents, fellows and postdocs including SEIU members not enrolled in a University Health Care Plan.
Program Incentives
Eligible participants can each receive up to $450 per calendar year ($450 per employee or $900 per employee plus an eligible spouse/domestic partner).
