3 Sessions

Determine what small nutrition and activity changes you can make to lower your cholesterol and reduce your risk of heart disease, heart attack, and stroke. Available in-person and by telehealth.

3 sessions

Discover how small changes to your diet and exercise can lower your blood pressure and reduce your risk of heart disease, kidney disease, and stroke. Available in-person and by telehealth.

6 sessions

Build a plan to manage withdrawal symptoms, prevent and manage relapses, and improve your overall health. Receive continuous support and encouragement to help you achieve your goal. Available in-person at Prince St. only, by phone, and by telehealth.

3 sessions

Receive personal support to address your nutrition goals and individual dietary needs. Learn how improving your nutrition can help you better manage existing concerns and prevent cancer, cardiovascular, metabolic, and other chronic diseases. Available in-person and by telehealth.

6 sessions

Develop a strength training program with expert supervision of goal-setting and form from a certified personal trainer. Sessions include assessments and guided personalized workouts. Available in-person only.

24 sessions – 18 group & 6 individual sessions

Achieve weight loss and improve your overall health with the expert guidance, support, and encouragement you need to make and maintain lifestyle changes. Adopt realistic strategies to improve your nutrition, increase your activity, overcome obstacles and stay motivated.

12 sessions

Receive continued support to stay on track after completing the Weight Loss program. Learn how to beat plateaus, manage stress, avoid food boredom, and keep workouts exciting and impactful to ensure long-term weight maintenance.

5 sessions

Learn the benefits and basic components of physical activity, discover how to prevent injury and manage recovery, and explore personal challenges to staying motivated. Program includes modified in-session workouts.

5 sessions

Review various dietary patterns, and discover how nutrition contributes to prevention of cancer, cardiovascular, metabolic, and other chronic diseases. Consider possible behavioral challenges to eating heart-healthy, and explore practical meal planning to help you incorporate heart-healthy recommendations.

4 sessions

Identify your stressors and learn how to reduce their effects to improve your relationships, find a healthier work-life balance, and refocus your goals and priorities.

6-8 sessions

Explore the studied practice of living mindfully, and discover how meditation can help you achieve more balance, stability and personal satisfaction. Program may include a day-long silent retreat.

Personal Wellness Coaching

Imagine what you can achieve with a personal wellness coach by your side! Personal wellness coaching for condition management is tailored to fit your individual needs and goals. You'll have the opportunity to work one-on-one with our team of wellness coaches including registered nurses certified in wellness, nutritionists, fitness specialists and stress and mindfulness experts.

In-person, telephonic and telehealth coaching options are available for most programs.

Learn how to avoid harmful triggers and properly use medications to breathe easier and feel better.

Learn how to avoid triggers for uncontrolled A-fib, and take necessary safety precautions to prevent complications.

Know your target blood pressure and cholesterol levels, and develop a plan to reach them. Create a diet and exercise plan to achieve and maintain a healthy weight. Learn how to manage your medications and keep your symptoms in check.

Identify lifestyle choices that will help manage symptoms of depression. Learn how to recognize triggers and to avoid choices that make symptoms worse. Keep blood sugar, blood pressure and cholesterol in check through proper diet and exercise. Learn to identify symptoms of hypoglycemia and hyperglycemia, and what to do when they occur.
