Our program begins with a comprehensive Personal Health Assessment (PHA) in which we assess each employee's cardiovascular health risks and lifestyle risks. Unlike other programs, we integrate the results of each step to get a clear picture of your employee's health, as well as what they are most motivated to improve. Through this process, we get the insight we need to better help each employee.

The PHA consists of three integrated steps:

  1. Questionnaire: Our evidence-based assessment of the employee's lifestyle and behavioral factors that contribute to health risks, as well as the employee's willingness to change their behavior. Using validated screening tools, we are able to identify lifestyle and behavioral health risks to employees.
  2. Biometric Screening: A one-to-one session with a registered nurse to measure values such as total cholesterol, HDL, LDL, triglycerides, blood pressure, blood glucose, BMI, heart rate, height, weight and abdominal girth.
  3. Biometric Health Coaching: A personalized discussion with a wellness nurse about results and health goals, allowing the employee to get tailored recommendations or referrals to lifestyle/condition management coaching programs.

UR Medicine Employee Wellness coaching programs are offered by a multidisciplinary team of registered nurses, certified wellness coaches, fitness specialists, and nutritionists. Coaching programs are evidenced based and designed to help employees achieve their goals: Lifestyle Management and Condition Management.

Lifestyle Management

Getting healthy means making better choices. Lifestyle coaching programs help employees make lasting lifestyle changes to sustain good health and reduce the risk of developing chronic disease. Participants work with our coaches to identify and establish attainable health goals. Our coaches also help your employee understand and successfully navigate personal barriers to enable lasting lifestyle change.

Condition Management Program

Integrated care is the best care.

Your employees' health is our top priority. This includes ensuring they are engaged with their primary care physicians. We support the PCP and patient relationship by including providers in the employee's wellness journey.

With the employee's consent, we will share wellness data with his/her primary care provider. This data includes biometric screening results, as well as condition management enrollment and completion updates. Cross referrals allow PCP referral into programs for eligible employees.

To make it even easier for employees to participate in our programs, we partner with payers. UR Medicine is a preferred provider for Excellus Health Plan, the largest insurer in the area. For eligible employers, we offer full coverage of our health assessments and condition management programs for employees on the health plan.

  • Personal Wellness Portal for each employee will provide customized program recommendations with enrollment options. In addition, the portal enables target e-mail outreach for recommended wellness coaching program(s).
